I'm wondering if your right foot (assuming you are right handed) is coming down late on your last turn? This could cause you to "hook" the hammer over to the left. It may be starting only slightly on turn 1 and by turn 2 or 3 your right foot is sort of "clunking" down late. Just an idea.
